In order for us to provide the best service possible during an emergency, it is beneficial for us to know some things about you. For example, if you have an alarm system at your residence it is helpful for the Village to know the names, addresses and phone numbers of any keyholders. If you have a security gate, it is good for us to know how to gain access without having to break through. It is also helpful to know whether or not emergency response personnel will encounter any pets in your home.

Information about existing health conditions may also assist us in providing emergency services. For example, if a resident normally uses a ventilator or portable oxygen, paramedics may want to know that in advance of arrival.

You can print the Resident Emergency Information Form or the Reverse 911 Form and return it to the Village, as indicated at the bottom of the form.
