forms may be picked up at the municipal complex, any U.S.
Post Office or may be obtained on the internet at www.travel.state.gov.
Beginning March 28, 2005 temporarily,applications will
only be accepted at the municipal complex on Mondays and
Tuesday between the hours of 8:30 am and 3:30 pm.
The applicant must appear in person
and provide:
Original or certified copies of proof of citizenship
(e.g. birth certificate or naturalization documents)
Picture identification (e.g. driver's license)
Two passport-sized pictures
Two checks – the Village is unable to accept
charge cards
For expedited service: 2 Express Mail Stamped

Children under
14 must be accompanied by BOTH parents
and minors aged 14 through 17 must be identified by one
parent.

Age 16 and Over - $67.00
Under Age 16 - $52.00
Additional Expedite Fee - $60.00
Village of Oak Brook Processing Fee - $30.00
Express mail - $16.25
PROCESSING
TIME:
Standard processing time is eight weeks
Expedited service (an additional $60.00 fee)
is three weeks.

downtown. An appointment must be made at 1-312-341-6020.
